This privacy statement provides information to our patients on how their personal information (which includes health information) is collected and used within the UNE Life Healthcare Centre, and the circumstances in which we may share it with third parties.
The UNE Life Healthcare Centre is a unit of, UNE Life and therefore the University of New England (University) and subject the Privacy Management Rule of the University. This Privacy Statement should be read in conjunction with the University’s Privacy Management Rule.
Your Personal Information
The Centre collects personal information about you in order to provide you with the full benefits of healthcare services. We also use your personal information for business activities directly related to your health care, such as financial claims and payment, practice audits and accreditation, and business processes (e.g. staff training).
The personal information we collect includes your:
- Name, date of birth, address/es, contact details
- Medical information, including, but not limited to, medical and family history, any care you may require, health risk factors.
- Medicare number (for identification and claims purposes).
- Healthcare identifiers.
- Health fund details.
Dealing with us anonymously
You have the right to deal with us anonymously or under a pseudonym, unless it is impracticable for us to do so, or unless we are required or authorised by law to only deal with identified individuals.
Using and disclosing your personal information
The Practice will not use or disclose any information about you without your consent unless:
- Required by law.
- We believe it necessary to provide you with a service that you have requested.
- In order to provide some services to patients, the Centre may be required to provide Patients’ contact details to third party suppliers of services. If any Patient does not want their contact details passed on to third parties for the purposes of health care, please contact the Centre.
- To lessen a serious threat to a person’s health or safety.
- To protect the rights or property of the Practice, any Practice user, or any member of the public.
